Black Women and Menopause: Breaking the Silence Around Disparities in Care

Introduction: The Quiet Storm So Many Black Women Face

For too long, the conversation around menopause has been whispered—if it happens at all. And for Black women, that silence is deeper, the disparities more complex, and the journey through perimenopause and menopause often more intense.

Studies show that Black women experience menopause earlier, suffer from more severe symptoms, and are less likely to receive proper care or diagnosis. But why is this happening—and what can we do about it?

In this blog, I want to pull back the curtain. As a physician and coach, and as a Black woman myself, I believe it's time to shine a light on what’s being overlooked in our health—and how we can reclaim our power and our peace during this pivotal time.

1. Earlier Onset and Longer Duration

Research shows that Black women tend to enter perimenopause up to two years earlier than white women, and remain in the transition phase longer. This means symptoms like hot flashes, mood swings, brain fog, and disrupted sleep may begin in the late 30s or early 40s, catching many women off guard.

🧠 Many of my clients thought they were just tired or stressed—until they realized it was perimenopause all along.

2. More Intense Symptoms, Less Support

Black women are more likely to experience severe and frequent vasomotor symptoms—that’s hot flashes and night sweats. Yet we’re less likely to be offered hormone therapy, non-hormonal alternatives, or even receive basic education about what’s happening to our bodies.

The reasons are layered:

  • Medical bias and gaps in cultural competency

  • Historic distrust of the healthcare system

  • Underrepresentation in clinical trials

  • Generational silence—our mothers and aunties didn’t talk about it

3. The Intersection of Racism, Stress, and Health

Chronic stress—including the weathering effects of systemic racism—plays a profound role in the menopause journey. Elevated cortisol levels, sleep deprivation, and trauma responses compound hormone imbalance, making symptoms harder to manage and increasing risks for heart disease, diabetes, and mood disorders.

This is not just about biology—this is about burden.

🔥 You can’t separate menopause from the context of a woman’s life. And for Black women, that life often includes extra layers of resilience... and exhaustion.

4. The Cultural Stigma: Who’s Talking About It?

In many Black communities, menopause is shrouded in shame or brushed aside as "just part of life." We are taught to be strong, silent, and selfless. But strength doesn’t mean suffering in silence.

We need safe spaces to say:

  • “I don’t feel like myself.”

  • “I’m tired of powering through.”

  • “My doctor isn’t listening.”

5. Rewriting the Narrative: What We Can Do

Here’s what I recommend to my clients and community:

Start Tracking Symptoms Early

Know your baseline—use tools like my Perimenopause Symptom Tracker to spot trends and advocate for yourself with data.

Find a Provider Who Listens

You deserve culturally competent, evidence-based care. Don’t settle for dismissal or vague answers.

Build a Sister Circle

Connect with other Black women going through this journey. Shared stories lead to collective strength.

Embrace Whole-Person Healing
